Successful Treatment of Right Parietal Haematoma Secondary

A 44-year-old young hypertensive male visited Max Smart Super Speciality Hospital, Saket, with complaints of headache, giddiness and weakness in the left upper limb since one day. On examination, the patient was sleepy but arousable and was following simple commands. GCS was E4V5M6, left upper limb power Grade 4, rest all limbs Grade 5, left-sided sensory inattention, agraphia, and astereognosis.

Cerebral Oedema and Hyperammonemia Treated Using CRRT

A 20-year-old male with no known co-morbidities came to the emergency department with a history of jaundice for five days, along with drowsiness for one day. He was admitted to Max Super Speciality Hospital, Saket, New Delhi, as a case of Acute Liver Failure (Hepatitis A virus-related) with Acute Kidney Injury (AKI), severe coagulopathy & Grade 3 Hepatic Encephalopathy secondary to Cerebral Oedema.

Diagnostic Dilemma For Periampullary Tumour Resolved & Treated

A 65-year-old man presented with high coloured urine for the last three months to Max Super Speciality Hospital, Mohali. There were no known comorbidities. On evaluation, he was found to have features of obstructive jaundice (Total Bilirubin-16 mg/dl) and elevated CA 19.9 (1068 U/ml). He underwent MRCP, which showed cholelithiasis, and dilated CBD with stricture at the lower-end. He was subjected to ERCP and stenting. EUS and side view endoscopy were also done, suggesting EHBO - distal CBD stricture and carcinoma gall bladder. Biopsy from the periampullary region did not show any malignancy. Bilirubin and CA 19.9 started coming down, but the diagnosis was still a challenge. Following this, the patient underwent a PET CECT scan which showed FDG avid GB fundus wall (SUV max 3.9).

Subserosal Fibroids Removal With A History Of Appendectomy & Caesarean Section

A 39-year-old female presented to Max Super Speciality Hospital, Mohali, with a complaint of pain in the abdomen. The patient had a caesarean section and appendectomy in the past. Ultrasound was suggestive of an 8.3 x 5.7 x 5.4 cm subserosal fibroid on the anterior wall. A diagnosis of Fibroid Uterus was made.

Treatment Strategies in No Option Critical Limb Ischaemia

A 70-year-old male, with a non-healing ulcer in the left leg for three months and in severe rest pain for four months, came to Max Super Speciality Hospital, Shalimar Bagh. He was not able to sleep and complained of continuous dull aching pain that got worse during the night. The patient had a history of smoking for the last 50 years, smoking 20-30 beedis per day. A CT angiogram was performed which showed very poor distal run-off. Still, an attempt for revascularisation was made, and tibial vessels were explored but found to be chronically occluded.

A Case of Acute Stroke with Symptomatic Severe ICAD

A middle-aged male came to the emergency of Max Super Speciality Hospital, Shalimar Bagh, with symptoms of gait imbalance, slurred speech, and nystagmus for four days. He consulted in some other hospital where he was managed conservatively. His MRI brain revealed acute cerebellar & pontine ischemic infarcts (posterior circulation stroke). MR Angio Brain & cerebral DSA suggested severe mid-basilar Intracranial Atherosclerotic Disease (ICAD) (>95% stenosis) & hence was put on dual anti-platelets.
