A 47-year-old patient from Uzbekistan was evaluated in her country by Dr. Amitabh Goel, Senior Director in the Department of Neurosurgery at Max Hospital Patparganj, Delhi. She had freezing of gait, along with extreme difficulty in walking and had to take 4 to 6 tablets of Syndopa Plus in one day to be able to complete her daily chores. Despite being on such high doses, she could not manage her household responsibilities.
Initially, she used to improve after taking these tablets but was still dependent on others. Besides, for the last 6 to 8 months, the effect of this medication had also started waning. She was evaluated in detail upon her arrival in India, including an MRI and PET CT scan which confirmed that Parkinson's Disease was lying on the right side of the brain. Neurology opinion was also sought with Dr. Vivek Kumar, Senior Director, Max Hospital, Patparganj and a combined decision was made to offer her Deep Brain Stimulation (DBS).
It is a technique where electrodes are passed into the brain via openings in the skull, targeting specific points called the subthalamic nuclei (STN). These worked very well after stimulation in the operation theatre and her tremors stopped. The position was confirmed based on a CT scan. The battery installed near her right shoulder stimulates the STN on the right and left side which helps her walk with fair speed. She is able to eat, button and unbutton, etc.
The patient had been following up for about ten days. She was down to almost one-tenth of her original medication requirement, and eventually she was able to stop her medicines completely.
